New Patient Registration
We welcome all new patients to the practice. Our list is OPEN. If you want to join the practice please complete a registration form which can be picked up from reception. Patients wishing to register at the surgery will be required to show the following documentation:
Proof of identity:
- Photo identity, such as a passport or driving licence
Proof of address:
- Such as a recent utility bill (gas, electricity, water, etc.) or a council tax bill
A birth certificate should be produced when registering children. If the child is of pre-school age please bring along their Personal Child Health Record (PCHR) - often called "the red book".
Once you have been accepted as a patient, your medical records will be transferred to us. You do not need to inform your existing practice that you are moving, although it may be courteous to do so.
When you register we will ask you to make an appointment with a practice nurse for a new patient health check. Receipt of your clinical records from your previous GP can take some considerable time. Not only does this appointment allow a relationship to be made with our surgery, it allows details of your history and current medication to be detailed. Should you require to see the doctor or nurse before your records arrive, then we at least have some information.
The practice boundary stretches for a three-mile radius around the Health Centre and covers Slaithwaite, Pole Moor, Golcar, Marsden, Linthwaite, Cowersley and Milnsbridge. In addition, the practice also accepts patients living in Helme and Meltham.
