Sharing Your Medical Record
Increasingly, patient medical data is shared e.g. between GP surgeries and District Nursing, in order to give clinicians access to the most up to date information when attending patients.
The systems we operate require that any sharing of medical information is consented to by patients beforehand. Patients must consent to sharing of the data held by a health provider out to other health providers and must also consent to which of the other providers can access their data.
National Data Opt Out – Type 1 and 2
The National Data Opt Out is about patients who do not wish to be contacted by the NHS for the purposes of planning or research. For example, if the NHS were planning to re-configure services, those patients who have opted out would not be contacted for their views.
A type 1 opt out can only be recorded by your GP Practice. A form will need to be completed and passed to the Practice to process. When you have completed the form please send to admin.slaithwaitehealthcentre@nhs.net
